GRP: Application Forms and Guidance

The following forms and guidance will be required to submit an application for Gender Recognition. Prior to completing and submitting an application for a Gender Recognition Certificate, all applicants should carefully read the Explanatory Leaflet – A Guide for Users.

The application forms provided are interactive, and can be completed online (or on paper). Completed forms should be printed, signed and posted to the GRP at the address shown on the About Us page.

If you decide to apply for a Gender Recognition Certificate, you should obtain copies of all the information and forms listed under the relevant application process below.

Overseas Application Process

*If you are applying via the overseas process, your acquired gender must have been recognised under the law of one of the countries or territories featured in this list.
